Since was changed the raise pitch for capital letters, NVDA play your error sound instead announce the capital letter.
It ocurs using any synth.

Doesn't occur with latest snapshot 4658 using any synth on my system. Festival, SAPI5 (various voices), Pico and Festival, espeak. Changing cap pitch in the voice dialog works as expected.

Replying to Ahiiron:

Several brazilian users have reported in portuguese list and me too.
Although I change the language, the problem persists.

Using any synthesizer, when spelling a capital letter, instead announce it, NVDA plays your error sound.
Also using review cursor, it ocurs when spelling a capital letter or a word containing one or more capital letters.
num2, num5 and num8 does not works when exists one or more capital letter.
The error sound is played instead.

Seems to be a sapi4 synth issue to me. I've tested with various languages and sapi5 and espeak, only happens on my systems with SAPI4. That doesn't explain "every synthesizer," but figured I'd still report it anyway as I still use TruVoice sapi4 sometimes.
