I tested with xp and windows 7.
The autorun is created, but nvda is not automaticaly started.

On XP, you will have to press enter once you plug in the USB stick. Can you confirm this?

On Windows 7, this does not work at all.

These are limitations of Windows for security reasons. We do need to document these somewhere, but there's nothing we can do to actually fix them.

NVDA no longer provides the option to creat autorun.inf (61c2d1c), as it doesn't work at all for USB drives now. Even on optical media, it still requires manual intervention from the user when the dialog appears.
