I've seen several cases of graphics labelled "mc-ref" in tables (and possibly elsewhere) in some PDF documents. IPDDomNode::GetName on these nodes often returns this, even though IAccessible::accName doesn't.

Given the difficulty of getting fixes into Reader in a reasonable amount of time, it's probably easiest to just work around this in NVDA, though I'll raise this with Adobe nevertheless.
