In Miranda file dialogues can not choose a file, because nvda plays only error sound and does not speak.
Example: When I want to choose a file for sending process or choose sound file for specific situation (incoming/outgoing message).

This is caused by event_NVDAObject_init in the miranda32 app module assuming that an object will be an IAccessible without checking first.

This should hopefully be fixed in r3604, but I'm unable to test it at present. Please confirm and close if fixed.
